Campus reopens for limited visitors

July 2 Update: Because Nashville has returned to Phase 2 of the Roadmap for Reopening Nashville, locker cleanouts have been postponed. Students & families should not enter school buildings.
With the city now in Phase 3 of the Roadmap for Reopening Nashville, students and families may return to campus to clean out their lockers on this schedule. Please visit usn.org/screening to complete a short health screening before entering USN buildings. Those unable to complete the form in advance must answer questions and have their temperatures checked in the 19th Avenue lobby. While on campus, everyone is required to maintain a social distance, wear a face covering over the nose and mouth, and wash/sanitize hands regularly.

Those who answer “yes” to any of the following questions may not enter USN buildings:

*Close contact is defined as being within approximately 6 feet of a person with confirmed COVID-19 for a prolonged period of time -OR- having direct contact with infections secretions of a person with COVID-19.

  1. Within the last 14 days, have you had or do you currently have a new onset of cough or shortness of breath?
  2. Within the last 14 days, have you had or do currently have a body temperature of more than 100.4 degrees Fahrenheit?
  3. Within the last 14 days, have you had or do you currently have chills, loss of taste and smell, or any other symptoms known as consistent with COVID-19? 
  4. Within the last 14 days, have you or anyone you have been in close contact* with had a positive test for COVID-19?
  5. Within the last 14 days, have you or anyone you have been in close contact* with been put in mandated quarantine for COVID-19? 
  6. Are you or anyone you have been in close contact* with awaiting test results for COVID-19?
