Share your vaccine status with the Health Team

USN's Health Team Director Margee Brennan encourages High School students to report their vaccination status as COVID-19 vaccine availability increases.
As COVID-19 vaccine availability increases, we encourage High School students who receive a vaccine to share that information with the Health Team by filling out the form here. Voluntarily reporting your vaccination status will help the Health Team to maintain the most up-to-date, complete records for contact tracing and similar COVID-19-related work. This information will not be shared outside of USN. 

On Monday, March 22, Gov. Bill Lee announced that Tennessee is updating coronavirus vaccine eligibility to all residents ages 16 or older by no later than Monday, April 5. The Pfizer vaccine is approved for ages 16 and older, while the Johnson & Johnson and Moderna vaccines have an age minimum of 18. No other vaccines are FDA-approved currently. Davidson Co. is offering vaccines only to individuals in categories through 2a/2b. According to the Tennessean, over 80% of vaccination appointments in rural, West Tennessee remain unfilled.
